As of the fourth quarter of 2023, Oak Haven Community Care Center was owned by Commcare Corporation, according to Centers for Medicare and Medicaid Services (CMS). The nursing home, located in Avoyelles County, has been owned by Commcare Corporation since March 1, 1994.

But when you need that type of care, there are limited options.”
